Please fill out this form and click on “Submit.” Your Library Card will be mailed to you at the address indicated. NOTE: Only individuals who live or work in Port Washington (NY) are eligible to receive a library card.

LIBRARY CARD RENEWALS are in person. Your library card expires every 3 years. Stop by the circulation desk with proof of address for an immediate renewal.
